Obtaining Copies of Death Certificates
Online Applications
You can apply online for a copy of a death certificate by visiting https://online.ebmd.rgd.gov.lk. For more information visit.
Copies of the death certificate should be obtained from the Divisional Secretariat corresponding to the place where the death occurred. If the relevant certificate is available in the database, certified copies of it can also be obtained from any Divisional Secretariat closest to you. Copies of certificates from the year 01.01.1960 to 31.12.2020 can be obtained through the database. (Whether it is available in the database can be checked and confirmed at the nearest Divisional Secretariat).
The required application form can be obtained from the District Registrar's Division of any Divisional Secretariat or from the website.
Fees Charged,
- When the death certificate number and the date of registration are known, the fee for one copy is Rs. 120.00.
- If the death certificate number and the date of registration are not known (as a search of records for a period not exceeding two years is required), the fee for one copy is Rs. 250.00
The relevant fee can be paid to the Divisional Secretariat or deposited into Account No. 7039827 of the Registrar General at the Bank of Ceylon (Battaramulla Branch). The receipt (Bank Slip) issued there, along with the duly completed application form, can be submitted to the relevant Divisional Secretariat to apply for certified copies.Payments made through Automated Teller Machines (ATMs) or online payments will not be accepted.
If it is required to have the death certificate sent by post, a stamped envelope bearing the address must be submitted along with the application form and the payment receipt as mentioned above.
If the death has been registered, the requested number of copies of the relevant death certificate will be issued according to the amount paid. If the death has not been registered, it will be notified via Form B 38.
Obtaining Translated Copies of Death Certificates

- From 9.00 a.m. to 2.30 p.m. on working days of the week.
- Closed on Saturdays, Sundays, and public holidays.

Documents to be Submitted,
- The duly completed application form
- The original copy of the death certificate to be translated or a certified copy obtained from a Divisional Secretariat
- If receiving by post, a stamped envelope bearing the recipient's address.
Translation Fees
- The fee for one translated copy is Rs. 600.00
- Any number of translated copies can be obtained
