Registration of a birth that occurred at home

The Grama Niladhari must be notified within 07 days of the birth.

For every birth , a report prepared in accordance with Form B23 must be sent by the Grama Niladhari to the Births and Deaths Registrar applicable to the place of birth. 

The birth is registered by the Registrar of the births and deaths division appointed for the area where the birth occured. 

The accurately completed Registration - Form CR01 must be submitted to the Births and Deaths Registrar relevant to the place of birth. The same registration – Form CR01 can be obtained from the Registrar relevant to the birth place or downloaded from the web-site. 

Persons eligible  to notify a birth 

  • Father
  • Mother 
  • Persons present at the time of birth  
  • Guardians

Doocument required 

  • Accurately completed Registation Form CR01
  • Photocopy of parents' marriage certificate 
  • Photocopies of parents' birth certificates 
  • Original and photocopy of the National Identity Card

If parents are unmarried, both parents must go to the births and deaths Registrar to include the father's details 

A birth can be registered free of charge within 03 months of its occurance. Registration after three months is treated as a late registration. Refferred details on past birth registration for that purpose.

After registration, A copy of the birth register is issued free of charge to the informant. 

A birth can be registered even if only the mother's known information regarding the date and place of birth is available.
